Medication Reconciliation (MedRec) plays a key role in ensuring patient safety. It is a systematic process in which healthcare providers partner with patients, families and caregivers to create a complete and accurate, up-to-date list of the patient’s medications at time of admission. This list is used to verify/validate admission, transfer and discharge orders to reduce medication errors and adverse drug events at all transitions of care.
