Immunization - Records Request

Provides records for most immunizations given through 糖心视频 (糖心视频) Public Health, Community Health or Travel Health Clinics.

Past immunization records and immunizations given elsewhere may or may not be available.

This may include immunizations given at:

  • a pharmacy
  • a doctor's office
  • an urgent care centre
  • an emergency department
  • a private travel clinic
  • any place outside of Alberta

Accessing immunization records from MyHealth Records:

Anyone age 14 years and older can register for and access their immunization records.

Parents / Guardians of children and youth under the age of 14 years or Guardians of youth over 14 years and adults can access immunization records through obtaining proxy access. For more information, please visit this website.

Accessing immunization records from a public health centre:

To make a request for anyone under 18, you must be the legal guardian. You may be asked to show proof that you are the guardian. If you are over 18, you must make the request yourself.

Accessing immunization records from MyHealth Records:

Anyone age 14 years and older can register for and access their immunization records.
