Outpatient follow up care for trauma patients admitted to and discharged from the hospital.

Follow-up clinic for patients seen by or admitted to trauma services.The clinic offers virtual or in person appointments as determined by the discharging trauma physician or nurse practitioner. The primary purpose of the appointment is to provide reassessment and education related to the patients admitting injuries and how they are managing post discharge.
